If you have a serious health condition or illness and are taking pharmaceutical medication, you must disclose all medications and other natural medicines that you are using to Rachel Collins during your consultation so that she can ensure that any natural or complementary medicine in the form of dietary advice, liquid, powder, tablets and/or capsules is safe for you to take. Each herb and nutrient has a therapeutic range and safe dosage which may vary from person to person depending on their health complaint or condition. For this reason, it is essential that you should never commence a natural medicine supplement liquid, tablet, powder, capsule or follow dietary advice without consulting with a professional to ensure your own safety and well-being and to significantly reduce any risk of negative interactions or side effects. You should also be aware that your decision to take a natural medicine supplement for your health condition, with or without a medication, is done so at your own risk.

Our Responsibility

We are responsible for providing in-clinic/online consultations, information sessions and products which are appropriate and customised to suit the unique health and wellness goals of our clients and customers. Rachel Collins is a degree-qualified health professional and full member of the Naturopaths & Herbalists Association of Australia (NHAA) and Australian Register of Naturopaths and Herbalists (ARONAH), as well as a Registered Fitness Professional (AUSRep – Fitness Australia) and abides by the associations’ codes of conduct. All clients and customers are treated with courtesy and respect.

We always consider the safety and efficacy of investigations and treatments, and nutritional, herbal or lifestyle interventions are not recommended unless clients have undergone a comprehensive consultation with our practitioner. We do not claim to diagnose or cure any medical condition or illness and we do not interfere with prescriptions or treatments recommended by our clients’ medical practitioners or health specialists. We may refer a client or customer to another health professional if the assistance required extends beyond our scope of practice. We recognise that every client or customer may have a different response to treatment and therefore do not guarantee results even when our practitioner has previously had a successful outcome with other clients or similar illnesses.

We will respond promptly to any inquiries received during business hours. For inquiries received outside of business hours, we aim to respond within twelve hours. Urgent matters which occur at any time will be prioritised and a response will be sent at our earliest convenience.

If a technical failure occurs during an online consultation, the practitioner will endeavour to rectify the fault. After three failed attempts, the practitioner will continue with the consultation via phone. Any time lost due to a technical fault will be added to the end of the session to ensure that you receive your allocated time.

Your Responsibility

It is your responsibility to fully disclose the following information to the practitioner prior to your treatment:

  • your health history and current symptoms

  • known allergic reactions that you have to substances, drugs, herbs, nutrients, supplements, chemicals, food and beverages, materials, and/or animals/insects

  • nutritional supplements, herbs or other natural remedies that you are taking, whether self-prescribed or provided by another practitioner

  • pharmaceutical or recreational drugs (legal or illegal) that you are taking or have used

  • your pregnancy status or intention to fall pregnant (if applicable)

  • any known impediments to your ability to follow a treatment plan or program

It is assumed that all information provided to the practitioner will be honest and accurate to the best of your knowledge. We cannot be held liable for any harm or damage caused as a result of deliberately misleading or incomplete information provided by clients and customers. Once treatment has commenced you must also report any changes to your health status, including improvement or exacerbation of existing symptoms, emergence of novel symptoms, potential reactions to treatment, changes in circumstance or health priorities, or any alterations to other prescribed medications and/or treatments provided by other health professionals.

It is vital that you follow all treatment instructions, whether verbal or written, and that you do not add, omit or modify any part of the practitioner’s prescription. If you have any reservations about the suitability of a treatment, dosage or regime that you have been given, you must make every attempt to contact the prescribing practitioner to discuss your concerns as they arise. When you agree to take a natural medicine supplement or to comply with other treatment recommendations, you do so at your own risk. Therefore, it is your responsibility at all times to ensure that you fully understand the benefits and possible risks involved.

All information and treatment guidelines given to you by the practitioner has been tailored to your circumstances and individual needs and is not intended as general advice. Under no circumstances should you ever share a prescribed treatment, in part or in its entirety, with another individual for the purposes of influencing or prescribing a health intervention. Any prescription, treatment or product offered to another individual presents a potential risk to that individual and may contribute to injury or death.

To achieve the full benefits of your treatment plan, you must attend appointments as scheduled by your practitioner. When sessions are repeatedly rescheduled or cancelled, treatment progress is disrupted, and it may mean that it is not the right time for you to commit to working towards your health goals.

You are expected to treat all Discover Health Pty Ltd staff with respect and courtesy at all times, regardless of the form of communication. We reserve the right to discontinue any appointment, transaction or conversation if we feel that your behaviour has become disrespectful or unacceptable. Whilst we encourage online interaction with us, we reserve the right to delete any comments made on our website (www.discoverhealthandfreedom.com) or any of our social media platforms if we deem them to be rude, disrespectful or otherwise unacceptable.

Payment Terms

A 100% of the consultation price is pre-paid to secure a booking. If you need to cancel your appointment, please provide a minimum of 24 hours’ notice. A cancellation fee of 50% of the consultation cost may be applicable if cancelling less than 24 hours before your scheduled appointment.

If a practitioner is unable to proceed with an appointment due to illness, accident or an environmental act outside of their control, we will contact you promptly and your appointment will be rescheduled to a future suitable time, at no additional cost to you. Booking an appointment or placing an order is an offer to enter into an agreement with us. No agreement is created until we have processed your booking or order and booking fee or payment.

All prices are shown in Australian dollars. Credit card payments can only be made using Visa or MasterCard and are made via Square. Online or phone consultations must be paid via credit or with debit card. Credit card details should never be sent via text, email or social media. Payment plans are available for programs and for events which last longer than one day. Payment plans must be requested. An up-front deposit is required, and the outstanding amount will be divided into equal instalments which are paid fortnightly or monthly, as agreed. An invoice will be set up with payment amounts and due dates clearly stated. Services will discontinue if money is outstanding and until the account is clear.

Shipping

Products may be collected from the clinic by arrangement. If required, items will be shipped via Australia post at cost to you. Packages are sent via Express Post and tracking numbers are available on request. The delivery fee is $15.00 Australia wide for all packages up to 1kg and will be added to the product invoice. For larger orders, postage will be as per Australia Post pricing at the time of ordering. Please be aware that once we have sent a package via Australia Post all responsibility for the delivery of your package belongs to Australia Post and Discover Health Pty Ltd will not be held responsible for lost, stolen or damaged goods.

Change of Mind

No refunds will be made for nutritional supplements or herbal products when you dislike their taste or texture, or if you no longer want a product for any other reason. However, under Australian Consumer Law, we agree to replace or refund any product which is found to be damaged or faulty, past its expiry date or that is proven to provoke a severe reaction upon use. If you suspect a problem with a product, please cease using it immediately and contact the clinic for further information.

Jurisdiction & Dispute Resolution

Discover Health Pty Ltd is located in Queensland, Australia.  This agreement is subject to the governing laws of Queensland and Australia, and these laws will apply to any disputes which arise.

If you have any complaints regarding our website, goods or services, you should contact us directly to discuss and resolve the dispute. If a resolution cannot be found, we will engage the services of a neutral third-party mediator. The costs of mediation will be shared equally between you and Discover Health Pty Ltd. Any residual issues will be dealt with under Queensland law.
