Our website (discoverhealthandfreedom.com) and the company Discover Health Pty Ltd (ABN 45 155 494 938) is owned and operated by Rachel Collins. Our online clinic is based in Main Beach, QLD 4217, Australia.
At Discover Health Pty Ltd, we are committed to protecting the privacy of the personal information we collect from clients and visitors to our website. Our Privacy Policy complies with the Australian Privacy Principles and the Australian Privacy Amendment (Enhancing Privacy Protection) Act 2012, which amends the Australian Privacy Act 1988.

a) Online booking forms on Simple Clinic;
b) Forms and information sent by you directly to us via email, text or postal services; and
c) Online and/or face-to-face consultations.
Sensitive information may be collected from children under the age of 16 under the following circumstances:
a) In the presence of their parents or legal guardians
b) With their parent or legal guardian’s full consent.
We are committed to securely storing and handling your sensitive information. We take reasonable and practicable physical, technical and administrative safeguards to protect your personal information from misuse, interference, loss, unauthorised access, modification and disclosure. Sensitive information is stored on password protected computers and only authorised Discover Health staff may access sensitive material. Some sensitive information may be stored securely online, or in the cloud through the healthcare client management software company, Simple Clinic. We will retain documents (including electronic documents) containing personal data:
(a) to the extent that we are required to do so by law;
(b) if we believe that the documents may be relevant to any ongoing or prospective legal proceedings; and
(c) in order to establish, exercise or defend our legal rights (including providing information to others for the purposes of fraud prevention and reducing credit risk).
Personal information that we process for any purpose or purposes shall not be kept for longer than is necessary for that purpose or those purposes. All archived sensitive information is securely destroyed after 7 years.
How we may use cookies

We will share information with our payment services provider only to the extent necessary for the purposes of processing payments you make via our website, refunding such payments and dealing with complaints and queries relating to such payments and refunds.

Complaints
If you believe there has been a breach of this Privacy Policy or the Australian Privacy Principles, or if you wish to request a change to your personal information, you may contact us by sending an email outlining your concerns to info@discoverhealthandfreedom.com and we will respond within 72 hours. Any complaints will be investigated thoroughly and the results of our investigation will be conveyed to you as appropriate.

We will not knowing disclose your personal information to overseas recipients unless they fall into the categories described above and for the purposes described above.
